Sports Physicals
The St. Sebastian Athletic Booster Club has arranged for Pre-Season Physicals for the 2013-2014 school year. The Akron General Sports Medicine team will come to Byrider as a convenience for our student-athletes. Emergency Medical Authorization and Participation forms are attached HERE. Once submitted, these forms cover all sports for 2013-2014.
Each physical will cost $25 -- this is Akron General's fee for this service. If you would like to participate, the dates and times are:
- Thursday, May 30, 2013 5:30-6:30pm
- Thursday, May 30, 2013 6:30-7:30pm
- Thursday, May 30, 2013 7:30-8:30pm
- Monday, June 3, 2013 5:30-6:30pm
- Monday, June 3, 2013 6:30-7:30pm
- Monday, June 3, 2013 7:30-8:30pm
Location: Byrider - Gymnasium Advance registration is required. Please email Chris Rademacher (rademac@earthlink.net) with the student names and which date/time slot you would like.
There are several other options for completing your pre-season physical paperwork if you aren't able to come to Byrider:
1. Take along the paperwork to your upcoming visit to the doctor.
2. Utilize the MinuteClinic at CVS: CVS/pharmacy #3306, 1949 West Market St, Akron, OH 44313, 330-867-5410, COST is $59.
3. Utilize the Take Care Clinic at Walgreen’s in Cuyahoga Falls: Walgreen’s store # 3572, 2645 State Rd., Cuyahoga Falls OH 44223, (330) 928-5444, COST until 9/30/13 is $39.
4. Schedule an appointment at Crystal Clinic in Montrose with the Sports Medicine Department; 330-344-4115, COST is $25.

- CYO Coaching Credentials: If you would like to coach or assist with any CYO athletic event, you need to be
certified. Please consider going through the process even if you have no immediate plans to coach or assist; you cannot help without these credentials. Contact the Athletic Director for details. We will post dates as they become available.
